NOTE: This trick does not work in Microsoft Vista and 7.It is even possible to customize the right click context menu and add new options or links to applications to it.
Follow the registry tweak below:
Step-1 Open the regedit.exe file.
Step-2 Search for the registry key:
HKEY_CLASSES_ROOT\Directory\Shell
Step-3 Within this key, create a new sub key having the name of the application that you wish to add to the Start button, right click on context menu. This is also the text that gets displayed on the context menu when the right click is made on the start button. For example, if you wish to add Notepad to the menu, the new sub key should be named Notepad.
Step-4 Now within this newly created sub key, create yet another sub key named Command. In the right panel, a new string entry shall automatically appear.
Step-5 Change the data value of this new string entry in the right panel of the registry to point to the full path name of the application that you want to add to the context menu. For example, in this case since we want to add Notepad to the context menu, the data value of the string entry should point to the path of the Notepad Application file.

Step-6 Exit the windows registry. You might need to restart your windows session for the changes to be implemented. The next time you right click on the Start button, a new menu item named Notepad shall also appear in the context menu.
It is also possible to remove items from this right click context menu by simply deleting the appropriate registry sub keys from the following registry key:
HKEY_CLASSES_ROOT\Directory\Shell
